When you mail your forms for certification approval (due by Dec. 1), they should be sent to this address, and you should always include your name and the name of your school. Remember to submit copies, not your signed originals. Copies will be returned to you with the requisite approvals, once any problems are ironed out. Copies of those approved forms should be kept in your notebook, for inspection at the fair, and should also be mailed with your registration materials when requested. |
